Annual membership dues are $15.00 per family or single due January 1st of each year.

To join WDMS or renew your membership:

1)	Print application form from this page (below) or click for PDF file of Membership Application/Renewal (the PDF file prints much better on single page).	2)	Make check payable to "WDMS"	3)	Mail check with completed form to WDMS Treasurer - address at bottom of form.
